I believe the distal finger strain means the ring or middle finger obviously. I don't know how you strain it though. He must have banged it somehow.

The distal bile duct is the far end of the cystic duct, the end away from the gallbladder. And the distal lymph node in a chain of nodes is the most distant one. The opposite of distal is proximal.

 

Edit: BA just said its the knuckle right under the fingernail which makes a lot more sense.
